United Healthcare/Optum Insurance is our ONLY insurance now being accepted by some providers at Arizona Counseling Centers. Full payment is due at the time of your session.
If you maintain alternate insurance, you may request a Super Bill at the time of service to submit directly to your insurance company for out-of-network reimbursement. Please check to see if you have any out-of-network benefits for individual/marital counseling. If you do have this type of coverage, your counseling may be covered in part by your health insurance or employee benefit plan. Please check your coverage carefully by asking the following questions:
Do I have mental health benefits?
What is my deductible and has it been met?
How many sessions per calendar year does my plan cover?
How much does my plan cover for an out-of-network provider?
What is the coverage amount per therapy session?
Is approval required from my primary care physician?
Cash, Checks, and all Major Credit Cards, Flexible Spending and Health Spending Accounts are accepted methods of payment.
When cancelling an appointment please contact your therapist directly to reschedule. Cancellations made less than 24 hours in advance, or not showing for a scheduled appointment, will result in a full session charge.
Effective January 1, 2022 under Section 2799B-6 of the Public Health Service Act, a ruling went into effect called the "No Surprises Act" which requires practitioners to provide a "Good Faith Estimate" about out-of-network care. At Arizona Counseling Centers, PC, we never want the cost of therapy sessions to be a surprise. We will provide all fees up front so that you can make the best decisions based on your needs and circumstances. Your therapist will collaborate with you on length of treatment, overall care and costs associated with those specific services. The majority of our Therapists are not contracted with insurance companies and are considered a “out of network” providers.
